Think I've cracked it. I have a habit of extracting all files from the .ap (don't ask) as I'd done for Kent High Speed. Although the Input mapper file was extracted to the input mappers folder (and modified there) it seems the game was reading the input mapper file directly from the .ap using the unmodified file. If I modify the file from within the .ap using 7-zip then all is well. Also if I delete the .ap file (after making a backup copy) from the Kent High Speed folder then the game reads the modified input mapper from within there and again the modifications work.
Trust this makes some sense, once again thanks for your assistance. Only goes to show that in my case a little knowledge is a dangerous thing!

Did the extracted folders actually fully mirror the archived structure?

Habit, initial ignorance and being a stubborn old Yorkshireman set in his ways I suppose Gary. I normally unpack the whole .ap file when I want to make some changes to files, although I now know it’s best to do it from within the .ap file itself. Yes, the unpacked .ap replicated exactly the file structure within. Having extracted it I normally remove the .ap file for safe keeping but on occasion reinstate it as some batch files for reskins require it to be present to work.
I’m now going to lock myself in a darkened room to repeat 1,000 times “I must not unpack .ap files” but I’m not promising anything!

Actually it's not. You should edit a copy of the archived file that's outside of the archive, in a mirrored folder structure. That file is then read by the game in preference to the one in the .ap archive.
The archive should stay as it came from Steam; a snapshot if you like of the files at whatever date it was installed.
