I have not spent enough time using the manual fireman controls in RW2 because I suck at it. Despite the auto fireman being an idiot it's still a better option all around because I'm not coordinated enough to be both a driver and fireman at once.

The way it works in real life is that it uses Bernoulli's principle (where pressure of any fluid is reduced when it moves faster) and the Venturi effect where the pressure energy of a gas can be converted to velocity energy and the reverse. Basically the fast-moving steam creates such a low pressure that it is nearly a complete vacuum, which sucks the water into the injector. The water then condenses the steam and passes through a diverging cone so the speed is greatly reduced and the pressure built up to a level greater than that of the boiler. There is then a simple valve that allows water to enter the boiler without allowing material from the boiler to flow back into the injector complex.

Hello briyeo1950,
Now somebody explain how the injectors work in RW2 as well as real life.
Can I suggest you have aread here about how the Injectors work:

http://en.wikipedia.org/wiki/Steam_injector

In RW the basically the same applies, you need to have the water valve open (wider open the valve the more water to the Boiler) and then open the Steam valve, In 'real' life the water has to be running first before you open the Steam valve and the water adjusted to get the Injector to deliver water to the Boiler, but this is not the case with RW and it works whichever is turned on first :-? .

This page about how the different parts of a Steam Loco work is also worth a read, if you are interested.
