Did you reference the cab camera blueprint in your engine blueprint? Have you re-exported the camera blueprint after making the changes? I don't think large numbers make a difference, in fact, you may want to look at smaller numbers to get it working, putting in a huge number like 20000 will probably not be accepted by RW as a valid location, it's way too far away. Try using coordinates from Blender by placing a sphere or something like that in the location you want the camera to be in and take the coordinates from it (that is, if you haven't already done that).
Unfortunately it's the same as adding couplers/child objects, you have to restart every time to see the updated version. The position you have now is nice, but maybe move a bit to the right? Or you could rotate the camera a tad to the right, I'd actually go for the 2nd option myself, I don't really like head on views, a bit of variation is always nice
I'd say its very slightly too low, the sun visor wouldn't have a big effect if you sat that low. The track looks very realistic from that height, though.
I think that's someone standing up, I think the drivers viewpoint would be in line with the 'Maximum speed 90mph', or just in line with the bit below it say SSF

Looks to be about eye height to me, as it is clearly looking over the shoulders of the driver and secondman
