i think the shape & textures are the most important, but they are all important

All of the above - it's a simulator and as such should simulate everything to the highest possible standard.
People aren't going to keep coming back to a model of a modern uk loco that looks like a shoe box, painted by a playgroup, that sounds like it's American and the cab puts you in completely the wrong place.
To even skimp on one detracts irreversably from the whole.

To single out one aspect would appear pointless.
My own opinion is that as IKB said, it must be accurately drivable.
The shape is important but good texturing can overcome minor faults. Not into phototexturing as it doesn't blend well with MSTS's rather crude scenery textures.
Sound is very important with diesels in particular, IMHO.
Cabview controls should be easily operable with a mouse, not cluttered or fussy. The FS cab will do me for me steam engines for that reason. With diesels, as long as the window shape appears reasonable, that's OK. Most of us havn't been in that many cabs and fewer have driven a loco.
We know what they look and sound like.
As an HST hurtles past, we know what they smell like but unless someone's come up with something really clever add-on, that's one pleasure we'll have to forego
